Coding Specialist

Position Summary
The HIM Coding Specialist will handle medical coding and data entry / abstraction for various types of Hospital visits (Inpatient, Outpatient, Ambulatory, Surgery, Emergency and Special Procedures). Below is a summary of duties and responsibilities.
Correctly assign ICD-9-CM primary and secondary diagnoses and procedure codes based upon documentation in the medical record
Correctly assign modifiers to charge master items and coder assigned CPT codes, as appropriate
Correctly assign CPT codes to outpatient procedures consistent with client contract and documentation in the medical record
Correctly assign E&M levels on ER facility and professional fees
Create compliant physician queries
Correctly and compliantly review claims for medical necessity
Update problem list consistent with client contract
Correctly assign present on admission indicators
Correctly and compliantly select DRG
Provide excellent customer service
Qualifications
(Must have at least one of these credentials)
Registered Health Information Technician (RHIT) with AHIMA
Coding Specialist Professional (CCA-P) (AHIMA) or Certified Professional Coder (CPC) (AAPC)
Certificated Coding Specialist – CCS – AHIMA
Experience
Minimum 3 years Health Information Management coding experience in an acute care setting required. Must have working knowledge of clinical data bases, data reporting requirements, and personal computer applications preferred. Excellent oral and written communication skills required.
Compensation: $17 - $30 per hr.

Responsibilities
This position is responsible for resolving claim edits and rejections related to facility coding. Responsibilities will include assigning codes and modifiers with ICD-10-CM, CPT and HCPCS Level II Codes.
Qualifications
AAPC – CPC, COC certification, or AHIMA – CCS certification

Comfortable using technology to perform responsibilities in a remote environment (computer, Internet, MS Office and VPN skills)
Good verbal and written communication skills
Must be able to maintain high quality standards of 95% or greater
Experience working with EPIC is preferred
Coders for this role must provide their own computer with a Windows Professional Based Operating System (MAC is not compatible) and dual monitors.

Outpatient Risk Coder – National
This position reports to the Manager of Risk Adjustment Coding. As a member of the Risk Adjustment team, the Outpatient Risk Coder works remotely in collaboration with hospitalists and hospital staff with opportunities for improved quality, risk adjustment coding performance. The Outpatient Risk Coder is a valuable resource in process improvement and identifying clinically appropriate risk adjusting conditions to capture. Must possess the ability to work independently with strong organizational, communication and interpersonal skills to support the management of multiple priorities, at multiple practice locations, with significant attention to detail for completion of both verbal and written external communications.
KEY RESPONSIBILITIES:
Review outpatient medical record information on both a retroactive and prospective basis to identify, assess, monitor and document claims and encounter coding information as it pertains to risk adjustment
Ensure that the diagnosis codes for each chronic or major medical condition have been captured and submitted within the permitted timeframe
Assess adequacy of documentation of claims and query outpatient provider claims to obtain additional medical record documentation or to clarify documentation to ensure accurate and appropriate coding
Audit provider documentation ICD-9 and ICD-10 codes to ensure adherence with CMS Risk Adjustment guidelines
Participate in departmental and physician network performance improvement initiatives.
Safeguards medical records and preserves the confidentiality of personal health information through the observance of physician network policies pertinent to the release of medical record information, record retention, and HIPAA privacy and security.
Performs other duties as requested.
REQUIRED KNOWLEDGE & SKILLS:
Familiarity and understanding of CMS HCC Risk Adjustment coding and data validation requirements.
Computer literacy of medical information system, records management software, encoders.
Advanced knowledge of CPT and ICD-10 coding required.
Proficiency in MS office (outlook, excel, word).
Knowledge of federal and state guidelines on all coding systems and sponsored programs
Professional interpersonal and organizational skills.
Attentive to detail and organization with ability to work with strict deadlines.
Strong interpersonal and communication skills: able to effectuate areas of opportunity to providers and practices as well as leadership.
Strong sense of diplomacy and ability to work under pressure.
EDUCATION/EXPERIENCE/LICENSURE/TECHNICAL/OTHER:
Minimum Education:
Associate degree in healthcare or equivalent work experience.
Certified Professional Coder (CPC) or Certified Risk Coder (CRC).
Minimum Experience:
2-3 years of experience in outpatient coding preferred.

Role and Responsibilities
Assigns ICD-10 and/or CPT/HCPCS codes, as appropriate and abstracts pertinent information from patient records
Ensures optimal reimbursement of all cases in compliance with CMS policies and procedures and Official Coding Guidelines
Implements medical centers physician query process when code assignments are not straight forward or documentation in the medical record is inadequate, ambiguous or unclear for coding purposes
Keeps abreast of coding guidelines and reimbursement reporting requirements. Brings identified concerns to the Coding Manager or Director of Coding Services.
Abides by the Standards of Ethical Coding as set forth by the American Health Information Management Association
Adheres to Official Coding Guidelines, CMS policies and regulations and Medical Centers policies and procedures
Ensures clients production and quality expectations are met
Communicates professionally and effectively
